Fatty Liver (MASLD) and Weight Loss in Illinois

Fatty liver disease, now called metabolic dysfunction-associated steatotic liver disease (MASLD), is very common and strongly tied to excess weight, type 2 diabetes and high cholesterol. Many people have no symptoms. Weight loss is one of the main treatments. This guide explains the basics.

What Fatty Liver Is

MASLD means extra fat is stored in the liver in someone with metabolic risk factors. In some people it progresses to metabolic dysfunction-associated steatohepatitis (MASH), which involves inflammation and scarring (fibrosis) and can lead to cirrhosis. Diagnosis involves blood tests, imaging and sometimes elastography or biopsy, ordered by your clinician.

Why Weight Loss Matters

Liver specialists' guidance recommends gradual weight loss for people with MASLD who have overweight or obesity. Losing roughly 5% of body weight can reduce liver fat, and larger losses of about 7% to 10% or more are associated with improvement in liver inflammation and, in some people, fibrosis. Limiting alcohol and managing blood sugar and cholesterol also matter.

What the FDA Has Approved

In 2025 the FDA granted accelerated approval to semaglutide (Wegovy) for adults with non-cirrhotic MASH with moderate to advanced liver scarring (fibrosis), together with diet and exercise. Other weight loss medications are not currently approved specifically for MASH, although weight loss itself helps the liver. Safety Considerations

People with advanced liver disease or cirrhosis need specialist care, and some medications are used with caution or not at all. Tell your clinician about all medications and supplements, and about any alcohol use. Rapid weight loss can occasionally affect the gallbladder, so report severe abdominal pain.

Can fatty liver be reversed?

Early fatty liver can improve substantially with weight loss and healthier habits. Scarring is harder to reverse, which is why early evaluation matters.

Is Wegovy approved for fatty liver?

The FDA approved Wegovy for MASH with moderate to advanced fibrosis in adults, alongside diet and exercise. It is not a treatment for every case of fatty liver.

Do I need a liver specialist?

Your primary care clinician will decide. People with fibrosis or abnormal testing are often referred to a hepatologist.
